I am submitting this application to the Virtual Staffing Team (VST) for the purpose of being placed as a work-from-home or ‘Virtual’ contractor. I understand that I will never be charged a fee by VST. I acknowledge that the use of this form, and my filling it out, does not indicate that any positions are open, nor does it obligate VST to further process my application. I hereby affirm that the information provided on this application (and accompanying resume, if any) is true and complete to be the best of my knowledge. I also agree that falsified information or significant omissions may disqualify me from further consideration for contract positions and may be considered justification for dismissal if discovered at a later date. I authorize all persons listed above (and on the accompanying resume, if any) to give VST, any and all information concerning my previous employment and education and any pertinent information they may have, personal or otherwise, and release all parties, such persons, and VST from liability for any damage that may result from furnishing same to VST. If placed as a contractor, I understand that I will be a contractor of the VST, and not of any VST client. If contracted, I further understand that my contracting position is not guaranteed for any specific time and may be terminated at any time for any reason. I further understand that a contract will exist between VST and each client to whom I may be assigned which will require the client to pay a fee to VST in the event I accept direct employment or contract with the client. I agree to notify my VST recruiter immediately should I be offered direct employment or contract by a client (or by a referral of the client to any subsidiary or affiliated company), either for a full-time, temporary (including assignments through another agency), or consulting positions within twelve months after the last day of any assignment period. VST is not a professional accounting firm. I will not render an opinion on behalf of VST on financial information (i.e. tax returns, financial statements, etc). I will not sign the name of VST or my name to tax returns or financial information for any client. I will not handle cash, checks or any other items of value without the prior written consent of my VST recruiter. Under no circumstances will I transport cash, negotiable or other valuables on behalf of any VST client or on behalf of VST. References: I understand that VST may obtain a consumer and/or investigative consumer report for contracting purposes that may include information as to my character, general reputation, personal characteristics, and mode of living, work experience and performance, along with reasons for termination of past employment. The report may also contain a records check of driving, criminal, credit, education, degrees, professional licenses and/or certification records depending on the position. By signing this application, I authorize the procurement of a consumer and/or investigative consumer report by VST as part of the pre-contracting background investigation and if hired, at any time during my employment.
